Parkview Occupational Health strives to make the DOT process easy for you.

To expedite the physical and clearance process, please prepare for your DOT or Bus Driver Physical by reviewing the grid below, click on the link to the form required and take it to your treating physician PRIOR to your Occupational Health appointment.

Our goal is to get you cleared, and keep you driving. Compliance with your treatment plan and completion of required documentation will help avoid holds. Please bring all required documentation to your physical appointment.

High Blood Pressure Send medication list with physical form. Compliant with treatment plan from physician, including taking medication as prescribed. Completion of DOT High Blood Pressure Clearance form. Controlled blood pressure at time of physical. At least two separate readings on two subsequent dates that are < 140/90. High Blood Sugar/ Diabetes Compliance with treatment plan. Send medication list with physical. Recent A1C lab test (within 3 months of physical date). Completion of DOT Blood Sugar Clearance form by treating physician. Recent A1C <9% plus completed Blood Sugar Clearance form. Click Here
Psychiatric Condition Compliance with treatment plan. Send medication list with physical form. Completion of DOT Psychotropic Clearance form by treating physician. Compliance with treatment plan. Completed DOT Psychotropic Clearance form. Click Here
Pain Medication / Other Substance Abuse Compliance with treatment plan. Send medication list with physical form. Completion of Substance Abuse Clearance form by treating physician or substance abuse professional. **Typically DOT does not allow bus drivers to operate a school bus while taking narcotics.** Compliance with treatment plan. Heart Condition / Stent / Angioplasty (Balloon) Compliance with treatment plan. Send medication list with physical form. Recent treadmill test post procedure. Completion of DOT Angioplasty/Stent Clearance form by treating physician. Compliance with treatment plan. Negative treadmill test 3-6 months post procedure or at least every other year per DOT recommendations. Completed DOT Angioplasty / Stent Clearance form. Click Here
Recent heart attack or acute myocardial infarction Recent doctor visit notes. Minimum of 2 month waiting period post acute MI. Will require full review of health history and may include hold. Click Here
Other disqualifying conditions: Monocular vision, someone with an implantable cardioverter defibrillator (ICD), someone taking narcotics, methadone, medical marijuana or anti-seizure medication for the prevention of seizures.
